Which medical specialties are safe from AI?
In the work a medical student is heading toward, 26 of a typical 40-hour week still need a person, by our task-by-task count. Tools could already take about 14 hours. Medicine is one of the most protected paths we score, and the protection is the kind you can read in law. A licensed physician must answer for the plan, the order and the consent.

Models are getting very good at the writing around that work: the note, the inbox reply, the prior authorization. Health systems are buying those tools fast, and every version we found keeps a physician reviewing and signing.
So the AI question for you is narrower than the internet makes it sound. It is which specialty you choose and what kind of resident you become in a hospital where the drafts write themselves. Your bigger risks sit elsewhere: whether you match, how much you borrow, and how the residency bottleneck plays out. The plan fits around clerkships, and it ends with something submitted under your name.
The hard truth
The new federal loan cap covers less than school costs.
From July 2026, new medical students can borrow at most $200,000 in federal loans. The typical four-year cost of attendance is higher than that, so many of you will need other money with fewer protections.

What stays yours
Taking a history and examining a patient
The history that matters comes from the patient in front of you, the family in the corner and what your hands find. A scribe records the conversation, but it does not decide which question to ask next.
Building the differential and committing to a plan
State medical boards license people, and in August 2026 the Federation of State Medical Boards wrote that physicians "should remain accountable for harms caused by inappropriate reliance on AI." The model can suggest, and the licensed physician answers for the result.
Telling a family bad news and getting consent
Informed consent requires a clinician who can answer the patient's questions and judge whether they understood. It is also the work patients most want from a person.
Procedures: lines, taps, sutures, deliveries
Nobody is close to a machine that places a central line on a crashing patient at 3 a.m.
Your board scores and clerkship evaluations
A proctored exam measures you alone, and program directors still screen on it.
How should a medical student plan with AI in mind?
Plan A is your best move in the field you are studying for. Most sessions in the 13-week plan build toward it.
- A
Pick your specialty by the decisions you want to own, and test it on rotation
Why it fits
Why you. You are choosing now, and every specialty you are likely weighing has strong demand behind it.
The gap. You need a written reason, one faculty conversation, and one week of logged tasks in the field.
The trade-off. It asks you to decide earlier than feels comfortable, and some advisors discourage that. You can still change your mind, and the written reason makes that easier.
Other moves that fit
These are also good bets in this field. Read them if Plan A doesn't suit you.
Become the student who checks AI notes and catches their mistakes
Why it fits
Why you. Residents will inherit scribe drafts, inbox drafts and decision support. Programs need people who check them well.
The gap. You need an error log, plus one small quality improvement (QI) or case project with a mentor.
The trade-off. A QI abstract carries less weight than bench research for the most competitive fields. It is also far more finishable on clerkships.
If radiology or pathology pulls you, learn to read alongside AI tools
Why it fits
Why you. If you enjoy the reading room, these fields have strong demand and the most AI tools.
The gap. You need a mentor in the department, a project touching imaging or pathology, and an away rotation in your fourth year.
The trade-off. The long view here is the least certain in medicine. Reading may look different in 25 years, and you would be betting on adapting inside the field.
What's a plan B for medical students?
Plan B is a job AI is creating that these skills already fit. One session a week in the plan trains you for it, so it is ready if your job shrinks or you want a change.
BPlan B, picked for this role
Expert AI trainer (contract)
Building a differential and committing to a plan is the clinical reasoning platforms pay medical trainees to check AI answers against.
Other options
Domain AI red teamer
Becoming the trainee who catches the machine on rotation counts as red-team practice, and a written report of it helps you stand out.
AI evaluation specialist
Taking a careful history teaches you which details matter, so you can design tests that show where medical AI misses them.
In your own plan you can switch to any of the 47 roles.
The first two weeks
The first 2 of 13 weeks. Most weeks have one 60-minute session and two 25-minute sessions, plus one 40-minute Plan B session. Weeks 6 and 12 are shelf or block exam weeks, with one 60-minute session and the Plan B session; move them to match your exam dates. Skip call and post-call days and move the session instead of doubling up. By week 13 you'll have an error log of where AI got cases wrong, a submitted abstract and a one-page specialty decision memo shared with your mentor.
- Week 1: Measure your week
What you do
- AList a typical week of your rotation or study block. Mark which parts AI already helps with.
- ARead your school's and your hospital's rules on AI tools for students. Write one paragraph you will follow.
- AList the specialties you are considering. For each one, write the decision you would be responsible for at 3 a.m.
- BTake the first half of Anthropic's AI Fluency: Framework & Foundations, a free online course. As you go, list which study and clerkship (clinical rotation) tasks you would hand to AI.
- Week 2: Answer first, then check AI
What you do
- AWork three practice vignettes, short written patient cases. Write your own differential (list of possible diagnoses) and plan first, then ask a school-approved AI tool and record every difference in a file called errors.md.
- ALabel each entry in your error log with one tag: missed finding, wrong dose, overconfident, or the tool was right and you were wrong.
- AWork two more practice vignettes the same way. Write your answer first, then compare.
- BTake Anthropic's AI Fluency for Students course. Read your school's AI policy alongside it.

Risks beyond AI: moderateTraining-pipeline risk
Residency slots, loans and payment rules will shape your next five years more than any model. Medicare-funded residency positions have been capped since 1997, federal loan limits for professional students changed on July 1, 2026, and roughly one in fifteen U.S. seniors did not match on the first try in 2026. Demand for physicians is strong. The bottleneck is training and paying for it.
Where this could be wrong
- The blend is ours. We averaged across specialties. A radiology week has far more interpreting and reporting, which raises the share of work AI assists with; a surgery week has far more procedures. Rerun the task list for your field.
- The evidence on scribes and inbox drafts is early, mostly single-system and mixed. UCLA's randomized trial found small time savings; Stanford found drafts did not save time. Savings may grow as tools improve, or stay small.
- We read radiology demand as strong for the next 15 years. The supply projections assume no AI productivity gains. If reading tools cut time per study sharply, demand per radiologist could soften later in your career.
- We could not confirm the status of the 2025 residency slot bill. If GME (graduate medical education) funding shrinks, the Match tightens regardless of AI.
